Why Rehab Is the Part That Makes It Last

By Dr. Jimmy Lopez, D.C.

Getting out of pain and staying out of pain are two different jobs, and most people only ever get help with the first one.

That is the gap rehabilitation fills. At Element Wellness Center in Point Loma we treat it as half the care, not as an add-on at the end.

Bending over to pick up your kids

We had one patient with such severe sciatica going down both legs, with numbness and tingling down both arms, that they could not even bend over and pick up their kids. They were having a hard time. They were missing work.

Through our corrective therapies, the way we adjust, and our shockwave therapy, not only were they able to restore function back to their legs and to their arms, but they were then actually taking their kids out, playing sports with them again, going on hikes again, just getting back to the things that a parent enjoys doing and wants to do, and playing an active role with their kid.

That last part is the part that matters. Nobody comes in here because they want a better range of motion score. They come in because there is a version of their life they are not getting to live.

We start by listening

Before any of that, we listen. That is genuinely the biggest thing we do, and it is the part that gets skipped most often everywhere else.

We want to hear what the problem is actually costing you, because that is what we are building toward. Someone who needs to get back on a job site has a different finish line than someone who needs to get back on a paddleboard, and a plan that ignores the difference is a generic plan with your name written on it.

What rehab actually does

An adjustment restores motion. Decompression takes pressure off a disc. Shockwave works on the soft tissue. All of that changes the structure.

Rehab changes what the structure can hold.

Muscles that have been guarding an injured area for months are not weak by accident. They are weak because your body shut them down to protect you. Once the joint moves again, those muscles have to be taught to take load, in the right order, or the old pattern comes straight back the first time you lift something awkwardly.

That is why people get a great result and then lose it eight weeks later. The correction was real. Nothing was built underneath it.

The order it happens in

Rehab that goes badly usually went out of order.

The first stretch is about calming things down and getting motion back into joints that have been locked up. Loading a segment that will not move is how people get hurt in a gym, and it is how people get discouraged in a clinic.

Once motion is there, the work shifts to waking up the muscles that shut down. That part is slower and less dramatic than people expect. It is also the part that decides whether you are back in here in a year.

Then it is capacity. Real load, in the patterns you actually use, so your body can handle a bad lift or a long flight or a weekend that got out of hand without falling apart.

It happens at home, not just here

The most important rehab you do is the rehab you do without us watching.

We send people home with tools, exercises, and equipment, because a few minutes a day between visits does more than anything we can do in an office an hour a week. Patients who do the home work keep what they gained. Patients who do not, generally do not.

Our goal is not to keep you coming in forever. It is to get you to a place where you have what you need to maintain it yourself.

How it fits with everything else

Rehab does not work in isolation, and neither does anything else.

For a disc problem, we may pair it with spinal decompression so the disc gets room while the surrounding muscles get strong. For soft tissue that has been irritated for a long time, SoftWave therapy helps the tissue respond. When a nerve is the driver, the pinched nerve side of things has to be addressed before loading anything.

If sciatica is what brought you in, the leg symptoms usually settle before the mechanics do, and that is exactly when people stop and exactly when they should not.

An active city is a rehab city

San Diego people do not want to be careful forever. They want to get back out in the sun, back on the water, back on the trail, back to pickleball.

That is a higher bar than pain relief. Getting a body ready to take load again is a training problem as much as a treatment problem, and it is the reason rehab is not the optional part.

Common questions about rehabilitation

How long does it take?

It depends on how long the problem has been there and how much function you have lost. A recent injury in an otherwise strong body moves quickly. Years of guarding around a degenerated joint takes longer, because there is more to rebuild. We will give you a real estimate after the exam rather than a number off a chart.

Do I have to come into the office for all of it?

No, and you should not want to. The visits are where we assess, adjust, and progress the plan. The exercises are yours. We send people home with equipment and a routine because a few minutes every day beats an hour a week in an office, every single time.

I already did therapy somewhere else and it did not hold. Why would this be different?

Usually because the structural side never got addressed. If a joint is not moving or a disc is compressed, strengthening around it fights the problem instead of solving it. We look at the structure first and build on top of it.

Where to start

If you have been through care somewhere before, got better, and then slid back, the rehab piece is usually what was missing.
